Learning Modalities for SY 2024-2025

  • CREATE Program (Blended Learning: Face-to-face classes with infusion of technology/LMS) for Junior Casa-Grade 12
  • PLAY Program (Home School Program) for Junior Casa-Grade 6

FOR NEW STUDENTS

Proceed to the Library of the High School Campus

Parent must bring the following documents:

  • Parent’s valid ID
  • For parents who will not be able to personally enroll their child, a signed authorization letter for guardians (with parent’s ID) must be presented/emailed. To view the format of the guardianship authorization letter, please scan:

  • If the parent is choosing CREATE Learning Program, a photocopy of the child’s vaccination proof (if available) must be submitted.
  • Child’s report card signed by the school principal
  • Birth certificate
  • Accomplished Pre – Registration https://bit.ly/MDSFRegForm
Step 1: Proceed to the admissions desk/enrollment kiosk to fill out the pre-registration form and for parent portal account creation.

  • Update student records
  • Signing of e-waivers
  • Uploading of COVID-19 Vaccination Card for Create (if available)
  • Upload Authorization of guardianship
  • Upload Parent’s Valid ID
Step 2:  Payment of assessment fee/ entrance examination to the cashier office.

The student applicant together with his/her parent/guardian shall proceed to the HS library and present the payment confirmation/official receipt to the Counseling & Testing Officer In Charge. The student will be given a  maximum of 3hrs. (for Grades 1 to 11) and 1hr. for (pre elementary) to accomplish the MDSF Entrance Examination/Placement Test. The result of the MDSF Entrance/Placement Test will be communicated to the parent/guardian within the day. 

*Student applicants residing abroad are exempted from the MDSF Entrance/Placement Test.

Step 3: Proceed to the Registrar’s Office to present the result of the entrance examination/assessment for printing of Registration Assessment Form (RAF) Window 1.

Submit any of the following:

  • Report card (if available)
  • Birth certificate
  • School ID

Important note: The student will only be officially enrolled upon submission of a report card to the Office of the Registrar. (hardcopy)

Step 4: Present the RAF to the Cashier’s office to proceed with payment of matriculation fees.
Step 5: Proceed to the School’s Bookstore to purchase uniform (optional)
Step 6: Present your Official Receipt (OR) to the ICT Dept for issuance of:

  • School ID
  • LMS account
  • Google Workspace account

 

How to reserve: A reservation fee of P5,000.00 may be settled in order to reserve an enrollment slot for SY 2024-2025. The said fee is deductible from the tuition fee payment upon enrollment and is only valid until June 15, 2024.

All new students/transferees are required to enroll onsite.

ONSITE ENROLLMENT

Step 1: Kindly go to www.mdsfonline.com and log in using your parent portal account. Make sure to upload your child’s vaccination proof (if available) when choosing CREATE Learning Program

The following shall be accomplished using the Parent Portal

A. Update student records
B. E-signing of waivers
C. Uploading of COVID-19 Vaccination Card for Create (if only available)
D. Upload Authorization of guardianship
E. Upload Parent’s Valid ID

Important note: The student will only be officially enrolled upon submission of report card to the Office of the Registrar. (hardcopy)

Step 2: Upon form submission, the status will be “pending”. The Registration Assessment Form (RAF) will be immediately available for viewing via parent portal.
Step 3: Payment of matriculation fees may be done via the following payment options
https://www.mdsf.edu.ph/other-payment-options. Parents may upload their deposit slip via Parent Portal or may send the deposit slip to finance@mdsf.edu.ph
Step 4: After payment verification, the status of the student will change into “Enrolled”.

Note: Official Receipts are available for claiming after confirmation of payment from M-Sat 8:00 am to 4:00 pm.

Step 5: You will receive an email from the ICT Dept. containing the details of the following

ID status (wait for the advise of the ICT Dept regarding the availability of ID)
LMS account
Google workspace account
SMS confirmation

 

How to reserve: Reservation Fee of 5,000 php is required to secure your child’s slot. Please be informed upon successful confirmed payment, your reservation is valid until June 15, 2023. No payment by June 15, 2024 will automatically result to a voided reservation. Reservation fee is deductible upon enrollment. It is non-refundable but can be applied to your child’s ledger upon enrollment at MDSF. Official receipt can be claimed from the Cashier’s office from Monday-Saturday from 8:00 am to 5:00 pm.
