Learning Modalities
Proceed to the Library of the High School Campus
Parent must bring the following documents:
- Parent’s valid ID
- For parents who will not be able to personally enroll their child, a signed authorization letter for guardians (with parent’s ID) must be presented/emailed. To view the format of the guardianship authorization letter, please scan:
- Child’s report card signed by the school principal
- Birth certificate
Step 1: Enrollment Kiosk.
Proceed to the admissions desk/enrollment kiosk to fill out the pre-registration form and for parent portal account creation.
- Pre-registration via Parent Portal
- Signing of waivers
- Filling out of Authorization of guardianship (in necessary)
- Present parent’s Valid ID (photo)
Step 2: Payment of assessment fee/ entrance examination to the cashier office.
The student applicant together with his/her parent/guardian shall proceed to the HS library and present the payment confirmation/official receipt to the Counseling & Testing Officer In Charge. The student will be given a maximum of 3hrs. (for Grades 1 to 11) and 1hr. for (pre elementary) to accomplish the MDSF Entrance Examination/Placement Test. The result of the MDSF Entrance/Placement Test will be communicated to the parent/guardian within the day.
Step 3: Entrance Exam/Placement Test.
Present the Official Receipt to the Counseling and Testing Officer in Charge.
Step 4: Registration & Assessment of Fees.
Submit the following:
- Birth Certificate
- School ID
- Report Card (can be submitted upon issuance by the previous school) (Student will be officially enrolled upon submission of report card)
- Recommendation Form (for high school only)
- Good Moral ( Grade 1- SHS)
Step 5: Payment of Fees.
Present the RAF to the Cashier’s Office.
Step 6: ICT Department.
Present your Official Receipt (OR) to the ICT Dept for issuance of:
- ID Printing for all students (CREATE & PLAY)
- Fetcher’s ID – required for CREATE Students (Pre Elem – Grade 6 )
- Fetcher’s ID or Gate Pass – for Grades 7 to 10 (parents should choose upon enrollment)
- Google Workspace account
Step 7: Books, Uniform and Instructional Materials
Purchasing of the items (optional)
- Uniforms
- Books
Claiming of the following items:
- Instructional materials
- Prime Math A&B (two set of books)
How to reserve: A reservation fee of P5,000.00 may be settled in order to reserve an enrollment slot for SY 2026-2027. The said fee is deductible from the tuition fee payment upon enrollment and is only valid until June 15, 2026.
Online Enrollment Process:
- Log in to your Parent Portal account.
- Click the “EnrollOldStudents” button.
- Wait for the school’s correspondence and assessment details.
- Upload your proof of payment through the portal.
Once your online enrollment is confirmed, you may proceed to the Enrollment Hub (HS campus) for:
- ID picture taking
- Claiming of school ID and other materials
Offline Enrollment Process:
